To obtain a medical second opinion, please complete the forms below, we will contact you shortly for more details and information. In the meantime, Please start gathering the required documents in the checklist.
The consultation is based on the information you provide. It is important that all documents and records are accurate and up to date. Please include all medical records that you would like reviewed and any questions that you would like the reviewing physician to answer.
The process begins once you have submitted all of the necessary documents.
If additional records or questions are provided after the process begins, there will be additional charges associated with reviewing that information.
What to expect
After all of the information has been received, medical second opinions generally require 7 to 10 calendar days, depends on the case and its requirement. We continually update you on the status of your request.
Typically, fees range from US$800 to US$1,500, depending on the complexity of the case and the preferred format of the medical second opinion. A more accurate estimate can be provided once all information is received. In some cases, a complete assessment may require more than one physician to review your records, which results in additional fees.
Any medical information that you provide is not returned to you upon completion of your medical second opinion unless you specifically request that it be returned. There is a US$75 fee for the return of any medical information. After six months of inactivity, all medical records are appropriately discarded.

Radiology
Please note that there are fees associated with review of radiology images. Radiology images
and corresponding report(s) are required for most clinical specialties. Images sent on CD should be in DICOM format. If possible, please send copies (not originals) of all documents and images. If sending films, please let us know if you need them to be returned.
Pathology –Please note that there are fees associated with review of pathology specimen and corresponding reports. Pathology specimen and corresponding reports are required by many clinical specialties. In order to avoid a delay at Customs, please write the following on the pathology slide/block commercial invoice: “Slides and/or paraffin blocks (and corresponding pathology reports) of non-infectious and non-contagious human tissue taken from the ____ (organ/tissue), which are fixed and/or preserved in paraffin.
This specimen has never been cultured. It is for review by the Department of Pathology at ———————, ——–, ——–. The value is US$1.00.” This declared value is for customs only.
